RIVERBANK PRACTICE PATIENTS PARTICIPATION GROUP

The Riverbank Practice Patient Participation Group (RPPPG) is undertaking a survey to obtain patients’ views on the current provision of services at the Practice. The survey will take place between

Monday 21st October 2013, through to Friday 15th November 2013, inclusive.

The RPPPG would be very grateful for the help of practice patients. A survey form can be obtained at the Riverbank surgery from Monday the 21st October 2013. Alternatively, a survey form will be available with repeat prescriptions, which patients collect from their chemist.

The completed forms, which are anonymous, should be returned to Riverbank Medical Practice where a collection box is provided for them, on, or before, Friday 15th November 2013.
